To explore the development of a rural Community Health Worker (CHW) navigation model that addresses hunger & food insecurity in Wilson County, Kansas.

Funding was identified as the primary barrier to launching a Community Health Worker program.
Stakeholders noted food and social service resources as difficult to navigate without guidance.  As a result of this, partners identified the development of a Community Resource Guide as a low-cost, actionable next step to ensure residents are informed about and connected to available food, health and social service resources.
