Resource Navigation

Project focus area

We’re trying to make progress on connecting ALICE and low-income households in Harvey County to the food, housing, health care, and financial supports they need to stay stable. By embedding a Resource Navigator at food pantries and other community hubs, we’re working to increase successful referrals and benefit enrollments and reduce the number of residents who are stuck moving from one crisis to the next.

Plan for action

Place a Resource Navigator at food distribution sites in Harvey County to provide person-centered support, connecting individuals facing food insecurity with essential services like housing, health care, employment, and SNAP enrollment.

Demographics

Persons with housing instability or who are unhoused, Persons with low income

Evidence

During this grant period, our Resource Navigator has provided one-on-one support to more than 120 individuals/households and documented over 250 referrals into the local safety net. Our new safety net tracking system (developed-and evolving-by our Navigator) allows us to monitor presenting needs, referral patterns, and barriers to service across partners, and early trends show especially strong demand for mental health and primary care (Prairie View, NMC Health), financial and basic needs support (Peace Connections, Salvation Army, local food pantries), and safety/trauma services (Safehope).

Qualitatively, clients tell us, “You people actually care,” and partners are seeing the difference: one adult education instructor described a student who was on the verge of dropping out of a rigorous technical program until meeting with our Navigator. After a single focused session that connected her to resources and prompted follow-up with her physician, the student returned to class “a different person” – engaged, organized, and thriving. As that instructor put it, this work is not just helping people get through the day; it is “literally saving lives” and changing the long-term trajectory for families.

Progress

  1. Set your project focus and actions, who you’ll engage, and how you’ll measure success—plus a simple timeline. Submit this setup to kick off.
  2. Share what’s done, what you’re learning, and any course corrections. Note any support needs to stay on track.
  3. Summarize outcomes and evidence (reach, partnerships, dollars), key insights and barriers, and recommended next steps. Completing this closes your Actioneer project.
